Latent profile analysis of active health behaviors among elderly hospitalized patients with hypertension

Abstract:Objective To explore the latent classes of active health behaviors and influencing factors among elderly hospitalized patients with hypertension, so as to provide the evidence for improving active health behaviors among elderly patients with chronic diseases. Methods Using convenience sampling, hospitalized patients with hypertension aged ≥65 years were recruited from the hypertension, geriatrics, and cardiovascular departments of three Class A tertiary hospitals in Urumqi City. Demographic data, hypertension duration, hypertension grade, and cardiovascular risk stratification were collected through questionnaires. Anxiety, depression, cognition, and active health behaviors were assessed with the Generalized Anxiety Disorder scale, Geriatric Depression Scale, Montreal Cognitive Assessment, and Active Health Behavior Scale for hypertensive patients, respectively. Latent profile analysis identified behavioral types, and multivariable logistic regression was used to analyze the influencing factors for active health behaviors among elderly hospitalized patients with hypertension. Results A total of 354 elderly hospitalized patients with hypertension were included, of whom 151 (42.66%) were males and 203 (57.34%) were females. The majority (233, 65.82%) were aged 65-<75 years. The mean score of active health behaviors was (105.79±5.82) points. Latent profile analysis classified the participants into two types: behavior-driven (76.55%) and responsibility-driven (23.45%). Multivariable logistic regression showed that, using the responsibility-driven type as the reference, patients with a middle school education level were more likely to belong to the behavior-driven type (OR=3.380, 95%CI: 1.244-9.185). In contrast, patients with a duration of hypertension exceeding 10 years (>10-20 years, OR=0.490, 95%CI: 0.273-0.880; >20 years, OR=0.383, 95%CI: 0.172-0.857), those with anxiety symptoms (OR=0.499, 95%CI: 0.283-0.881), and those with mild cognitive impairment (OR=0.456, 95%CI: 0.263-0.793) were less likely to belong to the behavior-driven type. Conclusions Active health behaviors among elderly hospitalized patients with hypertension exhibit two types: the behavior-driven type and the responsibility-driven type. The behavior-driven type was statistically associated with educational level, duration of hypertension, anxiety symptoms, and cognitive function.
