2020年广东省肿瘤登记地区结直肠癌流行特征分析

摘要 目的 了解2020年广东省肿瘤登记地区结直肠癌发病和死亡特征,为优化区域肿瘤防控策略提供依据。方法 通过广东省疾病预防控制中心肿瘤随访登记系统和全死因登记报告系统收集2020年广东省肿瘤登记地区结直肠癌发病和死亡病例资料,计算粗发病率、粗死亡率、35~64岁截缩率和0~64岁累积率,采用Segi's世界标准人口年龄构成进行标化。采用描述性流行病学方法分析不同性别、城乡和年龄结直肠癌发病和死亡特征。结果 2020年广东省结直肠癌新发病例14 771例,粗发病率为35.18/10万,世标发病率为24.84/10万,35~64岁截缩发病率为38.87/10万,0~64岁累积发病率为1.37%。死亡病例5 384例,粗死亡率为14.55/10万,世标死亡率为8.83/10万,35~64岁截缩死亡率为10.39/10万,0~64岁累积死亡率为0.37%。男性结直肠癌粗发病率和粗死亡率分别为40.35/10万和16.51/10万,高于女性的29.88/10万和12.54/10万(均P<0.05)。城市结直肠癌粗发病率和粗死亡率分别为38.94/10万和16.60/10万,高于农村的26.10/10万和10.42/10万(均P<0.05)。结直肠癌粗发病率随年龄增长总体呈上升趋势(P<0.05),80~<85岁组达峰值,为239.36/10万,85岁后明显下降;粗死亡率随年龄增长呈上升趋势(P<0.05),≥85岁组达峰值,为174.25/10万。结论 2020年广东省肿瘤登记地区结直肠癌发病率和死亡率高于全国水平,发病和死亡特征存在性别、城乡和年龄差异,需加强男性、城市和老年人群的结直肠癌综合防控工作。

AbstractObjective To investigate characteristics of colorectal cancer incidence and mortality in registration areas of Guangdong Province in 2020, so as to provide a basis for optimizing regional prevention and control strategies. Methods Data on incidence and mortality of colorectal cancer in cancer registration areas of Guangdong Province in 2020 were collected from the Cancer Follow-up Registration System and the All-Cause Mortality Registration Reporting System of the Guangdong Provincial Center for Disease Control and Prevention. The crude incidence, crude mortality, truncated rate for 35 to 64 years, and cumulative rate for 0 to 64 years were calculated, and standardized using the Segi's world standard population. Descriptive epidemiological methods were applied to analyze the characteristics of colorectal cancer incidence and mortality by different genders, urban/rural areas and ages. Results A total of 14 771 cases of colorectal cancer were reported in Guangdong Province in 2020. The crude incidence, world population-standardized incidence, truncated incidence for 35 to 64 years and cumulative incidence for 0 to 64 years were 35.18/105, 24.84/105, 38.87/105 and 1.37%, respectively. A total of 5 384 deaths of colorectal cancer were reported, with crude incidence, world population-standardized incidence, truncated incidence for 35 to 64 years and cumulative incidence for 0 to 64 years were 14.55/105, 8.83/105, 10.39/105 and 0.37%, respectively. The crude incidence and mortality were higher in males than in females (40.35/105 vs. 29.88/105, 16.51/105 vs. 12.54/105, both P<0.05). The crude incidence and mortality were higher in urban areas than in rural areas (38.94/105 vs. 26.10/105, 16.60/105 vs. 10.42/105, both P<0.05). The crude incidence of colorectal cancer initially increased with advancing age (P<0.05), reaching a peak of 239.36/105 in the 80-<85 age group, followed by a marked decline after 85 years. The crude mortality of colorectal cancer increased with advancing age (P<0.05), reaching a peak of 174.25/105 in the ≥85 years age group. Conclusions In 2020, the incidence and mortality of colorectal cancer in registration areas of Guangdong Province were higher than the national averages. There were differences in the characteristics of incidence and mortality among genders, urban/areas and age. Therefore, it is necessary to strengthen the comprehensive prevention and control efforts for colorectal cancer in males, urban areas, and the elderly population.
