儋州市食源性疾病沙门菌血清型和耐药分析

摘要 目的 了解海南省儋州市食源性疾病沙门菌血清型和耐药特征,为完善区域性食源性疾病防治策略提供依据。方法 收集2022—2025年海南西部中心医院和儋州市人民医院就诊的食源性腹泻患者粪便或肛拭子样本分离的沙门菌,经鉴定后采用玻片凝集试验鉴定血清型;采用微量肉汤稀释法检测沙门菌耐药性。结果 收集样本927份,检出沙门菌87株,检出率为9.38%。共鉴定出19种血清型,主要为鼠伤寒沙门菌、韦太夫雷登沙门菌和肠炎沙门菌,分别为35、17和6株,占40.23%、19.54%和6.90%。药敏试验分析结果显示,64株沙门菌株耐药,耐药率为73.56%;54株沙门菌多重耐药,多重耐药率为62.07%。沙门菌对四环素的耐药率较高,为65.52%。共检出38种耐药谱型,耐药重数分布于1~8重。优势多重耐谱型为氯霉素-复方新诺明-四环素-氨苄西林-舒巴坦和氯霉素-复方新诺明-四环素-链霉素-氨苄西林-舒巴坦,分别为8和5株,占12.50%和7.81%。结论 儋州市食源性疾病沙门菌血清型以鼠伤寒沙门菌、韦太夫雷登沙门菌和肠炎沙门菌为主,耐药和多重耐药严重,对四环素耐药性较突出,需加强耐药监测和抗菌药物合理使用。

AbstractObjective To investigate the serotypes and antimicrobial resistance profiles of Salmonella isolates from foodborne disease patients in Danzhou City, Hainan Province, so as to provide the evidence for improving regional prevention and control strategies for foodborne diseases. Methods Salmonella strains were isolated from stool or anal swab specimens of patients with foodborne diarrhea treated at Hainan Western Central Hospital and Danzhou People's Hospital from 2022 to 2025. After identification, slide agglutination tests were performed for serotyping. The microbroth dilution method was used to detect antimicrobial resistance of Salmonella strains. Results A total of 927 specimens were collected, and 87 Salmonella strains were isolated, with a detection rate of 9.38%. Nineteen serotypes were identified, dominated by Salmonella Typhimurium (35 strains, 40.23%), Salmonella Weltevreden (17 strains, 19.54%) and Salmonella Enteritidis (6 strains, 6.90%). Antimicrobial susceptibility testing showed that 64 strains were resistant, with an overall resistance rate of 73.56%, and 54 strains were multidrug-resistant, with a multidrug resistance rate of 62.07%. The resistance rate to tetracycline was relatively high at 65.52%. A total of 38 resistance patterns were identified, covering 1 to 8 antimicrobial agents. The predominant multidrug resistance patterns were chloramphenicol-trimethoprim-sulfamethoxazole-tetracycline-ampicillin-sulbactam and chloramphenicol-trimethoprim-sulfamethoxazole-tetracycline-streptomycin-ampicillin-sulbactam, identified in 8 (12.50%) and 5 (7.81%) isolates, respectively. Conclusions The predominant Salmonella serotypes isolated from foodborne disease patients in Danzhou City are Salmonella Typhimurium, Salmonella Weltevreden and Salmonella Enteritidis. The strains exhibit severe antimicrobial resistance and multidrug resistance, especially against tetracycline. It is necessary to strengthen antimicrobial resistance surveillance and promote rational use of antibiotics.
