| Trend of the female breast cancer incidence in Wenzhou City from 2015 to 2022 |

Abstract Objective To analyze the changing trend of female breast cancer incidence in Wenzhou City, Zhejiang Province from 2015 to 2022, so as to provide the evidence for optimizing breast cancer prevention and control strategies. Methods Data on female breast cancer incidence in Wenzhou City from 2015 to 2022 were collected through the Wenzhou Chronic Disease Collaborative Management System. The crude incidence, truncated incidence for 35-64 years, and cumulative incidence for 0-74 years were calculated. The Chinese population age-standardized incidence and the world population age-standardized incidence were calculated using the age composition of the standard population from the Fifth National Population Census in 2000 and Segi's world standard population structure, respectively. The average annual percent change (AAPC) was used to analyze trend in female breast cancer incidence from 2015 to 2022. Results From 2015 to 2022, the crude incidence, Chinese population age-standardized incidence, world population age-standardized incidence, truncated incidence for 35-64 years, and cumulative incidence for 0-74 years in Wenzhou City were 61.19/105, 42.73/105, 39.73/105, 104.50/105, and 4.23%, respectively, showing upward trends (AAPC=5.950%, 4.535%, 4.592%, 4.011%, and 4.897%, all P<0.05). The crude incidence (P<0.05), Chinese population age-standardized incidence, world population age-standardized incidence, truncated incidence for 35-64 years (P<0.05), and cumulative incidence for 0-74 years (P<0.05) among urban female were all higher than those in rural areas. From 2015 to 2022, the crude incidence, Chinese population age-standardized incidence, and world population age-standardized incidence among urban female showed upward trends (AAPC=2.899%, 1.837%, and 1.794%, all P<0.05), while the trends in the truncated incidence for 35-64 years, and cumulative incidence for 0-74 years were not statistically significant (both P>0.05). Among rural female, the crude incidence, Chinese population age-standardized incidence, the world population age-standardized incidence, truncated incidence for 35-64 years, and cumulative incidence for 0-74 years all showed upward trends (AAPC=6.981%, 5.457%, 5.648%, 5.172%, and 6.087%, all P<0.05). From 2015 to 2022, the crude incidence of breast cancer among female in Wenzhou City, urban areas, and rural areas first increased and then decreased with age. The crude incidence of breast cancer in Wenzhou City and rural areas peaked in the 55-<60 age group, at 136.07/105 and 128.16/105, respectively. The crude incidence of breast cancer among urban female exhibited two peaks: 142.08/105 in the 45-<50 age group and 161.95/105 in the 55-<60 age group. Conclusion From 2015 to 2022, the incidence of breast cancer among female in Wenzhou City was relatively high and showed an upward trend; the incidence was higher in urban areas, while the incidence of increase was more pronounced in rural areas, with the peak incidence occurring among female aged 55-<60 years.
