被动吸烟与妊娠糖尿病的关联分析

摘要 目的 分析被动吸烟与妊娠糖尿病(GDM)的关联,为制定GDM防治策略提供依据。方法 采用病例对照研究方法,选择2018年1月—2019年3月在宁波市妇女儿童医院住院分娩的GDM产妇为病例组,选择该院同期住院分娩的非GDM产妇为对照组。通过母子健康手册收集产妇基本信息;采用统一的调查表面对面调查产妇被动吸烟情况、流产史、家族史和孕次等资料;从医院电子病历系统导出产妇临床资料。采用Logistic回归模型分析被动吸烟与GDM的关联。结果 病例组672例,被动吸烟517例,占76.93%;对照组972人,被动吸烟600人,占61.73%。多因素Logistic回归分析结果显示,调整年龄、文化程度、民族、家族史和孕前体重后,被动吸烟(OR=1.873,95%CI:1.484~2.365)是GDM的危险因素;调整年龄、文化程度、民族、家族史、孕前体重、孕次、流产史和妊娠高血压后,被动吸烟(OR=1.906,95%CI:1.501~2.421)是GDM的危险因素。结论 被动吸烟增加GDM的发生风险。
